Common Laboratory Credentialing Challenges We Help Resolve

Incomplete applications, missing credentialing documents, incorrect provider information, and payer-specific requirements can slow laboratory enrollment and create unnecessary administrative work. Med Xpert Services helps address these issues through Lab & Pathology credentialing, NPI and taxonomy verification, CAQH and PECOS data checks, application follow-up, and support with revalidation deadlines and payer participation requirements.

Credentialing problems can also continue after enrollment when demographic, ownership, location, or billing information is not updated with payers. Our team helps identify and address these discrepancies, track payer responses, and support corrections that may help reduce enrollment-related claim delays, rejections, or denials. This allows laboratory billing and RCM teams to focus on revenue cycle operations while credentialing requirements remain organized and current.

Who we support

Laboratory Credentialing for Different Types of Laboratories

Each type of laboratory faces different enrollment requirements. Here is how our support fits yours.

Independent Clinical Laboratories

Standalone labs enroll as their own Medicare supplier and contract directly with Medicaid and commercial payers. We prepare and track every application and connect enrollment with your laboratory billing services.

CMS-855B · own Type 2 NPI

Hospital & Health System Laboratories

Multi-site labs need CLIA numbers, locations and provider data to match across every payer record. We keep hospital and outreach lab enrollments consistent as sites and providers change.

Hospital CMS-855A enrollment

Physician-Office Laboratories

In-office labs bill under the practice, so the CLIA certificate and lab details must be on file with each payer. We update payer records so lab claims are not rejected.

CLIA Waiver or PPM certificate

Diagnostic & Reference Laboratories

High-volume labs often work with many payers and receive specimens from several states. We manage multi-payer enrollment and track state licensure where it is required.

Multi-state, multi-payer

Specialty & Molecular Laboratories

Specialty testing can bring added payer requirements. For molecular laboratories, we also coordinate MolDX test registration where a Medicare contractor requires it.

MolDX where applicable

Pathology Laboratories

Pathology groups enroll as an organization and each pathologist enrolls individually. We handle CAQH profiles, reassignment and payer updates alongside your pathology billing.

Group 855B + pathologist 855I
Step one: CLIA

Which CLIA Certificate Does Your Laboratory Need?

Before a laboratory can enroll with Medicare or most other payers, it needs a valid CLIA certificate that matches the testing it performs. Labs apply on Form CMS-116, certificates are valid for two years, and the CLIA number appears on enrollment applications and lab claims.

Certificate of Waiver

Waived tests only

Laboratories that perform only waived tests.

Provider-Performed Microscopy (PPM)

Limited microscopy

Physicians, mid-level practitioners or dentists performing limited microscopy during a patient visit.

Certificate of Registration

Temporary, nonwaived

New labs performing moderate- or high-complexity testing until their initial survey.

Certificate of Compliance

Nonwaived, state survey

Moderate- or high-complexity testing after a state agency survey finds the lab compliant.

Certificate of Accreditation

Nonwaived, accredited

Nonwaived testing based on accreditation by a CMS-approved organization such as CAP, COLA or The Joint Commission.

Laboratories apply through the state agency where they are located. New York and Washington run CLIA-exempt state licensure programs, and other states, including California and Pennsylvania, also license clinical laboratories. Sources: CMS CLIA certificate guide, CMS exempt states.
Step two: Medicare

Medicare Enrollment Path by Laboratory Type

The Medicare application a laboratory files depends on how it is organized and who performs the testing.

Independent clinical laboratory

CMS-855B

Enrolls under its own Type 2 NPI with taxonomy 291U00000X (Clinical Medical Laboratory); a site visit may be required.

Hospital or health system laboratory

CMS-855A

Outpatient lab services are billed under the hospital’s own enrollment.

Physician-office laboratory

Practice enrollment

The lab’s CLIA number is reported on claims for lab services.

Pathologists

CMS-855I

Individual enrollment with reassignment to the group; the group enrolls on CMS-855B.

Reference, specialty & molecular labs

CMS-855B

Some Medicare contractors also require molecular test registration under the MolDX program.

What is laboratory credentialing?

Laboratory credentialing is the process of enrolling a laboratory with Medicare, Medicaid and commercial payers so it can bill for testing. It verifies the lab’s CLIA certificate, licensure, NPI, tax ID, ownership and location details against each payer’s requirements.

Does a laboratory need a CLIA certificate before payer enrollment?

Yes. Medicare and most payers require a valid CLIA certificate that covers the tests the laboratory performs, and the CLIA number is reported on claims for laboratory services.

Which Medicare form does a clinical laboratory use?

Independent clinical laboratories enroll on CMS-855B. Hospital laboratories are covered by the hospital’s CMS-855A enrollment, and individual pathologists enroll on CMS-855I with reassignment to their group.

How long does laboratory credentialing take?

Medicare contractors process most web-based applications within 15 to 85 days, depending on site visits and additional development. Medicaid and commercial payer timelines vary by state and plan, and commercial enrollment often takes several months.

How often does a laboratory need to revalidate?

Most Medicare providers and suppliers, including clinical laboratories, revalidate their enrollment every five years, and CMS can request it earlier. CLIA certificates must be renewed every two years.

Can Med Xpert credential our pathologists as well as the laboratory?

Yes. We support individual pathologist enrollment, CAQH profiles and reassignment to the group alongside the laboratory’s own Medicare, Medicaid and commercial payer enrollment.

What documents does a laboratory need for credentialing?

Most applications ask for the CLIA certificate and any state laboratory license, accreditation certificate, Type 2 NPI and taxonomy code, IRS EIN letter and W-9, ownership and managing employee details, the laboratory director’s license and CV, proof of the practice location, liability insurance, and banking information for EFT.
